Isotretinoin
Isotretinoin is a retinoid for systemic treatment of acne. Improvement of the clinical picture of severe acne is associated with suppression of sebaceous gland activity and reduction of their size, depriving Propionibacterium acnes of the main substrate for growth.
The drug inhibits the proliferation of sebocytes and acts on acne, restoring the normal process of cell differentiation.
The anti-inflammatory effect of isotretinoin on the skin has been proven.
The kinetics of isotretinoin and its metabolites are linear, it does not affect the activity of hepatic enzymes involved in drug metabolism.
In patients with acne maximum steady state plasma concentrations after fasting dose of 80 mg of isotretinoin were 310 ng/ml, reached after 2-4 hours. Plasma concentrations of isotretinoin are approximately 1.7 times higher than blood concentrations, due to poor isotretinoin penetration into erythrocytes.
Isotretinoin is 99.9% bound to plasma proteins. After oral administration, three main metabolites are detected in plasma: 4-oxo-isotretinoin, tretinoin (fully transretinoic acid) and 4-oxo-retinoin. After oral administration of radioactively labelled isotretinoin, approximately equal amounts are detected in urine and faeces. The terminal elimination half-life for the unchanged product in acne patients averages 19 hours.
Indications
Used for the treatment of severe acne (cystic nodules, conglobate acne or acne at risk of scarring) and acne unresponsive to other therapies.
Contraindications
- Pregnancy
- Breastfeeding period
- Liver failure
- Hypervitaminosis A
- Severe hyperlipidaemia
- Concomitant therapy with tetracyclines
- Hypersensitivity to the product or its components (including soya).
- Childhood under 12 years of age.
Side effects
- Blood system: anaemia, neutropenia, increased erythrocyte sedimentation rate, nasal bleeding.
- Digestive system: nausea, colitis, rarely hepatitis.
- Musculoskeletal system: myalgia, arthralgia, rarely hyperostosis. Dermatological reactions: skin rash, dermatitis, dry mucous membranes, erythema, sweating, paronychia, nail dystrophy, increased granulation of tissues in affected areas, rarely photosensitivity, vasculitis, thinning of hair.
- Sensory organs: photophobia, conjunctivitis, corneal clouding, decreased visual acuity at night, impaired hearing.
- Central and peripheral nervous system: often headache, rarely depression, suicidal tendencies, seizures.
- Allergic reactions.
Interaction
As tetracyclines may cause increased intracranial pressure, their use in combination with the drug is contraindicated.
Isotretinoin may weaken the effectiveness of progesterone preparations, therefore contraceptives containing low doses of progesterone should not be used.
Combined use with topical keratolytic or exfoliative agents for the treatment of acne is contraindicated due to possible increased local irritation.
